Choose the Kid's Membership if your child is 13 or younger, or the Adult Membership for anyone 14 and up. New and existing patients each have their own pricing.

Enroll and Pay Once

Pay the annual membership fee at our Niles office and your benefits become active right away. There is nothing else to sign up for or manage.

Use Your Benefits

Not an Insurance Plan: This is an in-house discount program offered directly by Smiles of Niles, not a third-party insurance policy, so there are no networks or annual claim limits to worry about.

One-Year Commitment: Membership is billed annually and renews each year unless you let our front desk know otherwise.

Cannot Combine with Insurance: Membership discounts are designed for patients who are uninsured or choosing not to use insurance for their care with us.

Family Discount Applies to Additional Members: The 5 percent family discount is applied when more than one household member enrolls in a plan.
